Value-based care has reached an inflection point. The Shared Savings Program is at record scale — 511 ACOs serving 12.6 million beneficiaries, with 82.8% in two-sided risk. Quality reporting has become a hard compliance wall: since performance year 2025, an ACO that fails to report eCQMs/CQMs receives an effective zero. ACO REACH sunsets at the end of 2026, the ten-year LEAD Model launches in 2027, and CMS interoperability mandates open new FHIR data feeds in the same window.
